The Local Care Team is made of representatives from all the child serving agencies in Caroline County (Caroline Human Services, Department of Juvenile Services, Caroline County Health Department Behavioral Health Clinic, MD Coalition of Families, Developmental Disabilities Administration, and Mid Shore Behavioral Health). If your child is between 5 and 21 and is displaying behaviors that concern you, struggling socially, being defiant or aggressive, displaying other risky behaviors or if you are struggling to coordinate services between agencies for your child who has an IEP or other diagnosis, the Local Care Team can help. The team will review your child’s needs and discuss resources and you will leave the meeting with a Plan of Care. A parent/caregiver can work with an agency already serving the child to make referral to the Local Care Team, which normally meets on the third Wednesday of each month or may contact the LCT Coordinator for assistance in making a self referral or accessing the appropriate agency to make a referral for your child.
